Chronic Care Management

Compassionate, Consistent Care for Your Pet’s Ongoing Health Needs

When your pet is diagnosed with a chronic or long-term condition, they need more than just emergency care—they need dedicated, ongoing support. Our Chronic Management Program at Burleson Animal Emergency Hospital provides specialized follow-up care designed specifically for pets managing conditions that require regular monitoring, medication adjustments, and long-term treatment plans.

Led by Dr. Jasmine Wilson, our Chronic Management veterinarian, this program offers your pet the continuity of care they deserve. Whether your pet is recovering from an emergency visit or managing an ongoing health condition, we’re here to ensure they receive consistent, personalized attention without the long wait times or scheduling challenges that can come with traditional veterinary clinics.

Our program bridges the gap between emergency care and routine wellness visits, providing a dedicated space for pets who need more frequent check-ins and specialized attention than a busy primary care practice can always accommodate.

What a Typical Chronic Management Visit Includes

  • Physical exam
  • Review of symptoms since last visit
  • Monitoring tests (bloodwork, urine, blood pressure, etc.)
  • Medication adjustments
  • Long-term care plan updates
  • Discussion time with the doctor

WHY WAS THIS PROGRAM CREATED?

  • Many pets with chronic or recurring conditions need dedicated rechecks and long-term management.
  • Your primary veterinarian may be fully booked or unable to see frequent rechecks.
  • ER hospitals are not designed for ongoing follow-up — they focus on medical emergencies.
  • Our Chronic Management program bridges that gap so your pet receives consistent, personalized care.

Meet Dr. Wilson – Chronic Management Veterinarian

Dr. Wilson graduated from North Carolina State University College of Veterinary Medicine in 2017. She went on to complete a Mixed Animal Internship at Alvarado Veterinary Clinic. From a young age, she knew she wanted to become a veterinarian. Although she briefly considered becoming a NICU physician during her undergraduate studies, her passion for the unique challenges of caring for multiple species ultimately led her to veterinary medicine.

With a strong foundation in emergency veterinary care, Dr. Wilson has spent her career helping pets during their most urgent and vulnerable moments. Through this work, she discovered the profound importance of long-term, relationship-based care — especially for patients managing chronic or complex medical conditions. This commitment inspired her to take on a central role in our Chronic Management Program, where she combines medical expertise with compassion and continuity.

Dr. Wilson believes that consistent follow-up, individualized treatment plans, and ongoing communication are essential to improving patient outcomes and easing stress for both pets and their families. Dedicated, attentive, and deeply invested in each patient’s long-term well-being, Dr. Wilson ensures that every animal in our Chronic Management Program receives thoughtful monitoring, reliable support, and the highest standard of care.
